Electrophysiology. City: Flagstaff State: AZ Start Date: Pending
Privileges End Date: TBD Description: Start Date: 2/2 Contract
Length: ongoing Shift Details: Feb 2-6 Feb 9-13 Mar 16-20 Apr 6-10
Apr 13-17 May 4-8 June 29-Jul 3 Jul 20-24 All shifts 7:00am-5:00pm
w 1 hour lunch 7a-5pm with 1 hour lunch Work Details: • Support
Staff: unknown • Amount of Coverage Needed per Month: weeks listed
• Patients per day: Clinic 20-22 patients • Patient Population/Age:
adult • Department Description: Cath Lab will be hospital consults
3-4 per day plus EP cases 4-6 a day depending on case length. •
EMR: Cerner • Hospital Bed size: 268 • Special Procedures: cardiac
electrophysiology specifically heart rhythm disturbances Ablations
- must show 100 EP cases in last 2 years in case logs with emphasis
on ablations Must be comfortable doing moderate sedation on their
own Technology - Abbott Ensite-X Mapping system must be comfortable
using must do TEEs and moderate sedation • Skills Required:
Albations Practice Setting: • Hospital • In-Office Clinic
Requirements: • Board Certification: BC or BE required • License
Required: AZ Licensed. Ok to license • DEA: AZ registered • CSR:
not required • Certifications Required: ACLS • Minimum Experience
Required: 1 year Credentialing Timeframe: 30 days OT: 1.5 normal
rate OT Billing Rule: after scheduled shift Holiday: 1.5x normal
