Job Description Job Description Travel Registered RN - ED Mental Health/RN Case Manager (Days/Nights) Mobile Health Team Inc. is seeking an experienced RN-ED Mental Health/RN Case Manager(Days/Nights) for a 13-week travel assignment in Tuba City, AZ. Assignment: Sunday -Saturday 10 shifts variable 7a- 530p & weekend rotation with call Start Date: 2/23/2026 Contract: 13 weeks EMR: Allscripts COVID & FLU Exemptions: May accept after the declination form is submitted Orientation: Every 2 weeks Location: Tuba City, AZ 86045 Fingerprinting must be submitted and cleared prior to start Hospital rental property very limited. Shared contractor housing only. No pets or family allowed per hospital policy. The Nurse Case Manager, Emergency Department (ED) is responsible for establishing, coordinating, and maintaining the process to increase patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum for the ED. The Nurse Case Manager collaborates with the patient/family, multidisciplinary team, physicians, community resources, and payers to ensure the patient’s progress and level of care are appropriately determined for the emergency episode of care and for the inpatient encounter to ensure that care is performed and facilitated efficiently and cost-effectively. Key Responsibilities: Works with an interdisciplinary team to assess care plans, monitor progress, and make recommendations to ensure safe and efficient hospitalizations. Responsible for evaluating patients in the ED for medical necessity and care coordination needs. Assesses potential admissions and works with advanced practice clinicians/physicians to determine appropriate admission status. Proposes and implements outpatient plans for safe discharge from the ED with physician approval. Coordinates care for patients with abuse, chronic pain, and frequent utilization issues. Compensation: Weekly Pay (hourly) Non-taxed Stipend (if eligible) Pay package details provided upon offer Requirements Minimum Requirements: Must have at least (2) two years of current care/case management experience in the Emergency Department Must have at least two (2) years of patient care experience (Hospital, Home health, Skilled Nursing) Must have at least 1 year of travel experience Associate’s or bachelor’s degree in Nursing A valid, current, full, and unrestricted Professional Nursing License to practice nursing in any state of the United States of America, the Commonwealth of Puerto Rico, or a territory of the United States Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment Must obtain Case Manager Certification (CCM or ACM) within 2 years from date of hire, if not already obtained Must be proficient with Electronic Medical Records, Outlook, Microsoft Office, Word, and Excel.
