Job Description:
General Position Summary: The staff Physical Therapist is
responsible for establishing quality care for the patients sent to
Summit Healthcare Regional Medical Center, supervision of physical
therapy aides and establishing appropriate goals and implementation
in patient care. Staff Physical Therapist I Licensed Physical
Therapist and able to practice in the State of Arizona. Able to
perform all duties and responsibilities as a physical therapist in
an inpatient and/or outpatient setting. Able to adequately
supervise support staff. Staff Physical Therapist II In addition to
the above duties and a minimum of 2 years experience as a physical
therapist, demonstrates expertise in one specialty area of physical
therapy including but not limited to Pediatrics, Gender Healthcare,
Geriatrics, Wound Care, Cardiopulmonary, Hand Therapy, Vestibular,
Lymphedema, and Neurological Rehabilitation. Certified in ACLS.
Staff Physical Therapist III Demonstrates expertise in patient care
with at least 10 years experience and current certification in one
or more specialty areas of treatment including but not limited to
Geriatrics, Orthopedics, Sports, Acute Care, Gender Healthcare,
Cardiopulmonary, Hand Therapy, Lymphedema, McKenzie, Neurological
rehabilitation, Pediatrics, Oncology. Excels in leadership roles
within the organization including program development. Essential
Functions / Major Responsibilities: · Perform patient evaluations,
formulate plans, provide direct patient care and supervise Physical
Therapy Aides and Physical Therapy Assistants. · Appropriately
document patient progress on a daily basis. · Appropriately
document patient evaluations and progress for all patients. · Send
initial evaluations and progress reports in a timely fashion. ·
Complete all duties of a Physical Therapy Aide and Physical Therapy
Assistant. · Attend in-service and continuing education. · Attend
other meetings per choice of department director. · Help in
implementation of policies and procedures. · Address pediatric,
adolescent, adult and geriatric patient and family concerns,
demonstrating specific knowledge of the needs, development and care
and/or treatment of the pediatric, adolescent, adult and geriatric
patient population. · Complete continuing competence for 2 year
renewal period. · Maintain current CPR certification and TB test.
Must have within 30 days upon hire and renewal. · Notify Board with
name and address change within 30 days. · Displays proper etiquette
and mannerisms that reflect the Keys to Excellence. · Promotes the
Patient Safety Standards as a core value of the organization.

courteous, make eye contact, smile, introduce yourself, address
people by name whenever possible. · Present a professional image …
apparel and appearance is appropriate, neat and clean, with name
badge highly visible. · Answer the telephone with a “smile”.

transfers as much as possible. · Listen to one another and to the
people we serve, then respond promptly and reliably. · Anticipate
the wants and needs of the people we serve … ask, “How can I help?”
· Effectively communicate with patients, families, and each other.
· Keep the people we serve informed about their care and treatment.
· Maintain a safe and clean environment. · Act to reverse negative
service situations using the “Anticipate, Apologize, Acknowledge,
Amend” process. · Respect the privacy and confidentiality to the
people we serve, our physicians/healthcare providers, and our
fellow employees. · Master the skills needed to do your best for
the people we serve. · Positively represent Summit Healthcare
Regional Medical Center in the workplace and in the community ·
Participates in departmental and hospitalwide informational
meetings and inservices, including staff meetings, hospitalwide
forums, and seminars. · Reviews department and hospitalwide
policies and procedures annually. · All other duties as assigned.
Specific Job Skills & Mental Activities: Biofeedback, Ultrasound,
NMES, FES, TENS, Cervical Traction, Moist Hot Pak Machine, Cold
Pack Machine, Whirlpool, Iontophoreses, UVC lamp, Paraffin,
Dynamometer and Goniometer, LLL (Low Level Laser light Therapy).
Exercise Machines: Hydrofitness, Stair Climber, Bikes, Treadmills,
Ellipticals, Nu-Steps, UBC, SciFit, Hoist machines (Leg Press, Lat.
Pull Down, Pulleys, Chest Press, Row, Knee Extension, Ab Crunches
Education and/or Experience: · Graduated of an accredited Physical
Therapy program (required). · Licensed in the state of Arizona
(required). · High School diploma or equivalent (required). ·
Current CPR/BLS certification (required). Physical Demands & Job
Conditions: Exert up to 50lbs. of force occasionally, and/or up to
20 lbs. of force frequently, and/or up to 10 lbs. of force
constantly to move objects. The worker is subject to contact with
chemicals, contact with infectious fluids, heavy lifting, constant
standing, and encounters with upset/disturbed individuals. Physical
motions include finger dexterity, standing, walking, stooping,
crawling, talking, reaching, feeling, sitting, bending, kneeling,
climbing, grasping, listening/hearing, handling, lifting up to
50-100 pounds, and repetitive motions of the hands, wrists, and
feet. This is a safety sensitive position.
