seeking a Physical Therapist for outpatient setting Monday -
Friday. Open to New Grads $10k Sign On Bonus - Relocation

Major Responsibilities: Perform patient evaluations, formulate
plans, provide direct patient care and supervise Therapy Services
Techs and Therapy Assistants within scope of practice.
Appropriately document patient progress on a daily basis.
Appropriately document patient evaluations and progress for all
patients. Send initial evaluations and progress reports in a timely
fashion. Complete all duties of a Therapy Services Tech and Therapy
Assistant within scope of practice Attend in-service and continuing
education. Attend other meetings per choice of department director.
Help in implementation of policies and procedures. Address
pediatric, adolescent, adult and geriatric patient and family
concerns, demonstrating specific knowledge of the needs,
development and care and/or treatment of the pediatric, adolescent,
adult and geriatric patient population. Complete continuing
competence for 2 year renewal period. Notify Board with name and
address change within 30 days. Adhere to applicable State of
Arizona licensure statutes and applicable national standards such
as but not limited to NBCOT, ASHA as required to maintain
professional designation. Displays proper etiquette and mannerisms
that reflect the Shine Behavior Standards. Promotes the Patient
Safety Standards as a core value of the organization. Secondary
Functions: Treat the people we serve as guests … be courteous, make
eye contact, smile, introduce yourself, address people by name
whenever possible. Present a professional image … apparel and
appearance is appropriate, neat and clean, with name badge highly
visible. Answer the telephone with a “smile”. Identify yourself and
ask how you can help the caller. Eliminate transfers as much as
possible. Listen to one another and to the people we serve, then
respond promptly and reliably. Anticipate the wants and needs of
the people we serve … ask, “How can I help?” Effectively
communicate with patients, families, and each other. Keep the
people we serve informed about their care and treatment. Maintain a
safe and clean environment. Act to reverse negative service
situations using the “Anticipate, Apologize, Acknowledge, Amend”
process. Respect the privacy and confidentiality to the people we
serve, our physicians/healthcare providers, and our fellow
employees. Master the skills needed to do your best for the people
we serve. Positively represent Summit Healthcare Regional Medical
Center in the workplace and in the community Participates in
departmental and hospitalwide informational meetings and
inservices, including staff meetings, hospital wide forums, and
seminars Reviews department and hospitalwide policies and
procedures annually. All other duties as assigned. Specific Job

equipment Education and/or Experience: Graduated of an accredited
Physical Therapy program (required). Licensed in the state of
Arizona (required). High School diploma or equivalent (required).
Current CPR/BLS certification (required within 30 days of hire
date). Employee to maintain certification without gaps in order to
maintain Success Sharing eligibility. Suspension without pay will
occur 31 days after expiration. Physical Demands & Job Conditions:
Medium Exert up to 50lbs. of force occasionally, and/or up to 20
lbs. of force frequently, and/or up to 10 lbs. of force constantly
to move objects. The worker is subject to contact with chemicals,
contact with infectious fluids, heavy lifting, constant standing,
and encounters with upset/disturbed individuals. Physical motions
include finger dexterity, standing, walking, stooping, crawling,
talking, reaching, feeling, sitting, bending, kneeling, climbing,
grasping, listening/hearing, handling, lifting up to 50-100 pounds,
and repetitive motions of the hands, wrists, and feet. OSHA
Exposure Category: Involves exposure to blood, body fluids, or
tissues.
